Akira Health is an Accountable Care Organization (ACO). ACOs are networks of doctors who are contracted with Medicare to help improve the quality of care that his/her Medicare patients receive as well as to reduce overall healthcare costs by providing more effective care.
In order to increase the quality of care that Medicare patients receive, Akira Health provides social work case management, as an extension of the doctor’s office, to patients of its member physicians. Therefore applicants must be comfortable working within an interdisciplinary medical team. The Social Work Coordinator is responsible for reviewing the patient’s medical record, formulating a care plan, and calling the patient. Home visits may also be required. The Social Work Coordinator provides ongoing resources, interventions, referrals, and support to patients. In addition, the Social Work Coordinator communicates all updates to the patient’s physician, nurse, and/or other providers as needed.
QUALIFICATIONS
B.A. in Social Sciences or Health Sciences (MSW student or graduate preferred)
Ability to work within an interdisciplinary team
Excellent verbal and written communication skills (previous case management experience is a plus)
Experience with the aging population (experience working with Medicare patients is a plus)
Ability to work independently and proactively, with minimal oversight
This is a start-up company with room for growth and innovation, therefore flexibility and openness are required
Familiarity with conducting assessments
Time management skills
TYPICAL WORKING CONDITIONS
This position is largely sedentary. May require some travel within Santa Clara County during work hours.
